After clicking on the link below, you will be prompted with a dialog box asking you to enter your user name and password . You user name is your NYC account number. The format is letter (capitalized) followed by a dash (-) then the number which is 3 digits. Add a 0 (zero) before if you have a 2 digit number. For example, Q-1 becomes Q-001, Q-11 becomes Q-011. Your password is your last name, capitalized as you would normally but with any spaces removed additionally any suffix is dropped. So "Jones, Jr." becomes Jones.
